Community-based Case Manager  (1) Position

 

The Community-based Case Manager assists the Eskasoni Case Management Team in developing and implementing comprehensive mental health programs and services for referred clients within the community of Eskasoni and is accountable to the Director of  Eskasoni  Mental Health Services.

The successful applicant should possess the following:

A Bachelor’s level degree in Psychology, Social Work or related profession, with a minimum of three (3) years experience;

Knowledge, and experience of First Nations political, governance, and community structures;

Strong interpersonal skills and the ability to work effectively with a wide range of individuals in the organization, communities and federal and provincial representatives.

Superb verbal and written communication skills with an ability to communicate complex information;

Superb clinical, consultation, assessment and counselling skills;

Experience with Crisis Response Teams and the Case Management concept;

Ability to work independently and expeditiously under tight timeframes and competing priorities;

Perform other related duties as requested by the coordinator;

Respect confidential nature of the business and management of a Case Management Team;

Be a registered member of their professional organization, where applicable;

Must be able to work within a multi-disciplinary context  (school, justice, addictions, hospitals, mental health settings);

Undergo a Police Check and Sexual Abuse Registry Check.

Ability to speak Mi’kmaq considered an asset

Valid driver’s license, current vehicle insurance coverage and access to a reliable vehicle are essential

Salary:Commensurate with qualifications.
